Description:
Sodium Chloride 3% solution is a hypertonic saline compared to body fluids, meaning it contains a higher concentration of sodium chloride than normal saline (0.9%).

Dosage Form and Packaging:
Clear, colorless injectable solution, supplied in 100 mL packs.

Drug Classification:
Hypertonic saline solution.

Indications:
Used for the treatment of severe hyponatremia when neurological symptoms such as seizures, muscle stiffness, or decreased level of consciousness are present.
It is also used to reduce intracranial pressure (ICP) in cases of head injury, helping to lower brain swelling and pressure inside the skull.

Dosage and Administration:
Dosage depends on the severity of hyponatremia and the presence of neurological symptoms.

  1. To increase serum sodium by 4–6 mEq/L during the first six hours.
  2. Recommended dose: 2 mL/kg over 10–20 minutes. For adults, up to 100 mL over 10 minutes can be administered.

Contraindications:

  • Hypertension (high blood pressure)
  • Edema (fluid retention)
  • Heart failure
  • Kidney failure

Precautions:
Should never be used without medical supervision.

Drug Interactions:

  • Potassium-sparing diuretics (e.g., spironolactone): may alter electrolyte balance.
  • Lithium: sodium chloride 3% may reduce lithium serum levels.
  • Corticosteroids: may cause sodium and fluid retention.

Adverse Effects:

  • Hypernatremia (high sodium levels)
  • Coma
  • Increased blood pressure
  • Fluid retention and edema
  • Pulmonary edema

Store below 30°C, protect from freezing and direct sunlight.
