Product Name: Mannitol 20% IV Solution
Dosage Form & Packaging:
A clear, colorless, sterile, non-pyrogenic, hypertonic solution (1199 mOsmol/L) for intravenous injection.
Supplied in a PVC infusion bag with an injection port and infusion port, free from antibacterial preservatives.

Drug Classification: Osmotic Diuretic

Indications:

  • Treatment of increased intracranial pressure (ICP) caused by cerebral edema.
  • Reduction of intraocular pressure in acute glaucoma.
  • Induction of diuresis in acute kidney failure.
  • Assisting in toxin elimination in cases of poisoning.
  • Reducing the risk of renal failure before dialysis.

Dosage and Administration:

Administer only by slow intravenous injection or continuous IV infusion.
Use an infusion set and IV pump for precise administration.

Patient Monitoring:

Monitor electrolytes, fluids, blood pressure, and kidney function during treatment.
Before use, assess the patient’s renal, cardiac, and pulmonary status.
Correct any fluid or electrolyte imbalance prior to administration.

If crystals form in the solution, warm it gently and shake until fully dissolved; use only after the solution becomes clear and transparent.
Use immediately after opening; discard any remaining solution.

Indication                                       Recommended Dose

Decrease ICP                                  0.25–1 g/kg over 30–60 minutes

Decrease intraocular pressure  1.5–2 g/kg before eye surgery

Diuresis                                            0.5–2 g/kg, depending on patient condition

Contraindications:

Do not use in:

  • Congestive heart failure
  • Pulmonary edema
  • Anuria due to acute renal necrosis
  • Severe dehydration
  • Intracranial bleeding

Warnings:

  • Use with caution in patients with severe heart or kidney dysfunction.
  • Extravasation (leakage into surrounding tissue) may cause inflammation and thrombophlebitis.

Precautions:

Extreme caution in:

  • Renal, hepatic, or cardiac insufficiency
  • Electrolyte disturbances
  • Very high intracranial pressure

Drug Interactions:

  • With digoxin: may increase digoxin toxicity due to hypokalemia.
  • Avoid use with aminoglycosides, other diuretics, or cardiac glycosides.

Side Effects:

  • Chills, nausea, vein inflammation at injection site
  • Fluid and electrolyte imbalance
  • Rapid or excessive infusion may cause mannitol accumulation, extracellular fluid overload, hyponatremia, hyperkalemia, acidosis, tachycardia, anuria, or oliguria.

Pregnancy:

Category C — use only when potential benefits justify potential risks, under medical supervision.
In life-threatening conditions (e.g., cerebral edema or acute glaucoma), mannitol may be used cautiously.

Breastfeeding:

Mannitol may pass into breast milk; use only with medical advice and caution.

Storage Conditions:

Store at 15–30°C.
Protect from freezing and direct sunlight.
Do not use cloudy solutions or those containing undissolved crystals.
